Please read our revised classified ad policy before submitting your ad. Effective December 2010, there is a charge for all ads placed in the February 2011 newsletter and beyond.

We are revising this submission form to include payment capabilities. In the meantime, please call the office at (800) 235-3502 to pay by Visa or MasterCard, or mail your check to the OVMA at 1880 Lancaster Dr. NE, Suite 118, Salem, OR 97305. Prepayment is required.

Please include your clinic/hospital name, as well as the OVMA member's name if you are submitting an ad on a member's behalf.
